Where to Enter a Fee Waiver Code

Fee waiver codes are generally entered on the final page of the form in a field labeled "Fee Waiver", "Office Use", "Administrative Use" or something similar. If you cannot find this field on the last page of the form, you may wish to check the first page instead. Clicking the "Submit" button after entering your fee waiver code should bypass the payment screen, so you should not need to enter your payment information (such as a credit card number).
