If the email address on your account is a generic or shared email (for example, a shared departmental email address) and someone from your institution or organization previously used it to sign up as a recommender at CollegeNET, both the account and your current recommendation form will display their name.
Other reasons might be that the applicant entered your name incorrectly.
You can correct the name displaying in the recommendation form by updating the account name within the Manage Account section of your profile.
